Here are a few points that might help...

Don't be surprised if you are flamed to hell and back for posting a survey. Many of us are sick of being treated as lab-rats.

Cross-posting is considered very poor netiquette. 

Thank you for having another option for gender than male or female

Not all people of black ancestry are African-American. Some of the other ethnicities you've listed are too specific or not specific enough, and the list is woefully incomplete.

"Does your avatar represent who you are?" is an enormous question, arguably requiring a detailed survey in its own right.

You don't make any allowance for people who swap between multiple avatars on one account, or multiple accounts with different avatars.

Don't create a required question ("What are the parts that represent yourself?") that might have no relevant answers for some respondents. 

The final question is so open that the only truthful answer is 'maybe'.
