Do you still have the hud? If you do, have you tried to reset it? If the hud is lost too, what were you doing and where when it disappeared? Was it during a crash?

There used to be a nasty SL bug which meant that, if you edit your dance hud in any other way than

• Detach the hud back into your inventory (don't drop it)
• Rez it from your inventory and edit it
• Take it back into your inventory and wear it from there (don't wear it from the ground)

You risked losing it.  

That was supposed to have been fixed a couple of years ago, but it may have found its way back in, and since we can't see other people's recent bug reports in the jira any more, there's no way of knowing.   It would certainly do no harm to submit a bug report, though, with full details.

The other possibilty that occurs to me is that there's a feature in some TPVS, "Restore to last position" or something like that.  That used to be in the official viewer but was removed because there are so many circumstances in which it can go wrong and cause permanant inventory loss -- and using it in connection with any sort of hud is one of those circumstances that's almost certain to go wrong.
