Should you be having problems with that there could be a number of reasons:
With the 2.6 viewer in Basic mode you can't edit your appearance or use inventory, so you can't change/remove clothes
With the Advanced mode or another viewer the quickest way to find all the things you're wearing is to type 'worn' in the inventory search box. Right-click and 'remove' each of these.
Starter avatars have 'underwear' painted on the skin itself so you can't accidentally get nude. You will need to get another skin which does not have this.
Occam's Razor (House version): the simplest explanation is invariable ... someone screwed up.

4. Be sure that you are in the privacy of your own home or are on a sim where nakedness is allowed (not a PG sim).
